MediTrack — Digital Patient Management System

Overview

MediTrack is a comprehensive hospital information platform designed to replace fragmented paper processes with a connected, digital-first OPD workflow. It enables quick registration, QR-coded slips for one-scan patient retrieval, real-time access to longitudinal history, and structured e-prescriptions generated at the point of care. The system bridges the gap between front-desk operations and the doctor’s console, ensuring that records are available instantly anywhere in the hospital network. With modules for investigations, diagnoses, and medicines master tables, MediTrack standardizes documentation while remaining practical for busy, high-volume public facilities. The result is faster patient handling, fewer lost files, and better continuity across visits.

Clinical Problem

Manual registration counters, handwritten slips, and physical files create bottlenecks and inconsistencies in busy OPDs. Charts are misplaced or incomplete, audits become difficult, and doctors cannot immediately see a patient’s full course of care. Without digitization, actions such as searching old records, reconstructing drug history, or sharing information between departments become time-consuming. This slows decisions, increases the risk of duplicated tests or prescriptions, and reduces overall efficiency. A lightweight but robust system that preserves the practicality of printed OPD slips while enabling instant digital retrieval can transform day-to-day care delivery.

Methodology

1. Front desk registers patients using name/ID/Aadhaar; prints a QR-coded OPD slip.
2. At the doctor’s desk, scanning the QR instantly loads the patient chart, history, and demographics.
3. Doctors record diagnoses, investigations, and structured e-prescriptions via a medicines master table.
4. Integrated pediatric dose calculator (Dose-Cal) supports safe paediatric orders.
5. Advanced search by name/ID/Aadhaar/date; deploy on a local server with optional cloud backup.
